Section C: The Rare Disease Mandate: A Partnership for Impact

Our core proposal is a partnership to integrate rare disease identification into your community work. Your teams are uniquely positioned to see the children and adults who have been left behind. SFRD will provide all the training and backend support, allowing you to save lives without diverting from your primary mission.

Agreement to the Mandate: "Yes, our organization agrees in principle to adopt the mandate of identifying, flagging, and referring probable rare disease patients to the SFRD Network.
